Clinical Case Management & Family Support Services

Case Management Services During Treatment

Throughout the treatment process, the Case Manager serves several vital functions.

These functions include:

Serving as the main liaison between the loved ones of the client and the clinical/treatment team.
Helping the client and the clinical team develop a personalized aftercare plan.
Identifying appropriate facilities and levels of care throughout the treatment journey.
Consistently providing client-centered support.
Coordinating care with therapists, psychiatrists and primary care physicians.

Case Management is also crucial to the successful implementation of aftercare planning. The Case Manager makes sure the needs of each individual are being met consistently. Additionally, the Case Manager provides accountability and support surrounding the components of long-term sobriety. Aftercare plans are developed by the client, Case Manager and treatment facility prior to the discharge date.

Case Manager Services & Aftercare

As your Case Manager, I will serve all of the above-listed functions while you or your loved one is engaged in treatment. I also provide Case Management services during aftercare, including:

Recovery Coaching & Mentoring

Recovery begins when a person commits to addiction treatment — however, this is truly only the first step on what will become a lifelong recovery journey. I am available to help my clients and their loved ones navigate early recovery; including the period of time immediately following treatment. A Recovery Mentor is essentially a qualified person who provides support, guidance, and professional insight over the course of the recovery process. My personal experiences allow me a unique level of compassion and understanding.

Integration into Peer Support Groups

For most individuals, immediate integration into a peer support group means the difference between relapse and continuous sobriety. 12 Step support groups like Alcoholics Anonymous and Narcotics Anonymous can be extremely beneficial, providing an increased level of accountability and support from others who are in a similar place in their lives — or who have been previously, and have come out the other side. However, 12 Step programs are not the only option. I will help you find a peer support group that works for you.

Relapse Prevention Planning

The period of time immediately following treatment is, by and large, the most vulnerable for those who are new to sobriety. It can be difficult to navigate an entirely new way of life while feeling every emotion fully — both positive and negative. Most of my clients learn relapse prevention techniques while they are engaged in treatment, and I help them continue honing these new skills while adding additional techniques to their personal repertoires. It is crucial for those who are new to recovery to know how to handle difficult situations and emotions as they arise. Whereas they previously turned to drugs or alcohol as a means of coping, they will now utilize a range of healthy, effective and productive coping mechanisms.

Family Support Services

It can be difficult to know just how to help a loved one who has recently completed an inpatient treatment program. Additionally, family dynamics which were negatively impacted by active addiction will still require attention and reparation. I offer several support services to the loved ones of my clients, including individual therapy, family therapy, and comprehensive recovery coaching.

I also offer family support services outside of the addiction recovery realm. When one member of the family is struggling with something, everyone is impacted in one way or another.

Being a person in long term recovery and trauma survivor, I am perfectly positioned to make my patients feel safe that they can discuss their challenges openly. My goal is to help people overcome barriers and evolve into the person that truly want to be
